The effectiveness of mandibular infilltration compared with mandibular block technique in the restoration of mandibular second primary molar

Background & Aim: Injection is the first stage in almost all dental procedures and causes the most negative reaction of children. Inferior alveolar nerve block is common injection for primary mandibular molars. This technique involves some disadvantages especially in pediatric dental treatments. The two important items are more stress and long time anesthesia that may make the child bite his/her tongue and lips. Regarding these problems, many alternative techniques are proposed instead. In this study, the effectiveness (rate) of infiltration anesthesia compared with block in filling the mandibular second primary molars in 5-8 years old children.

Method & Materials: A randomized controlled clinical trial in which 34 5-8 years old cooperative children that were required to second molar restorations in both sides were selected. During one session common block injection and during next session infiltration were done. Eye movement, hand and body movement, oral complaints and crying during cavity preparation were observed and recorded.

Results: None of 34 children have hand and body movement, oral complaints or crying in both infiltration and block anesthesia. Only one case had eye movement during block injection but not in infiltration.

Conclusion: Results show that there were no differences among 4 reactions evaluated. According to the advantages of infiltration injection we can replace it with block anesthesia for filling primary mandibular (second) molars with 100% success.
