Evaluation of flow cytometry for detection of Squamous Cell Carcinoma in Saliva

Background & Aim: Squamous cell carcinoma (SCC) is the most common form of malignancy of the oropharyngeal region, including more than 90 percent of all malignancies of this area. Different methods have been used by researchers in order to achieve diagnosis, screening and determine prognosis. Nuclear DNA ploidy is known to be related to the survival rate for this tumor but there is no previous study in relation to tumor screening and detection according to DNA ploidy in saliva.

Method & Materials: This study was designed to evaluate the role of flow cytometry (FCM) of saliva in early detection of oral SCC. Twenty patients with primary oral SCC and 30 healthy controls were studied and flow cytometry was carried out using desquamated epithelial cells in saliva. The results were compared to histopathologic findings, as golden standard. Controls were matched with patients according to the most important risk factors of oral SCC, such as age, sex, smoking and alcohol consumption.

Results: There were 17 cases with DNA aneuploidy and one with tetraploidy in patients and only 2 individuals with aneuploidy in control group. The frequency of non-diploidy was different between two groups (90.0% vs. 6.7%, p<0.0001). The mean of S-phase fraction was 26.4% in patients with SCC and 15.5% in controls (p<0.05). The mean of DNA index was 1.40 (ranged from 1.00 to 2.71) in patients group.

Conclusion: The sensitivity and specificity of FCM for detecting primary oral SCC was 90.0% and 93.3% in saliva, respectively. The accuracy of the test was 92%. This test can be used as a helpful diagnostic test adjacent to routine pathological examinations for patients.
